The Great Start Readiness Program is funded by a grant from the Michigan Department of Education. The State has identified factors that may affect your child's education at any time in his/her kindergarten through 12th grade school career.

Great Start Readiness Application

Examples of these factors include:

• Your child has a delay.

• Your child has challenging behavior.

• Your family's primary language is not English.

• You do not have a high school diploma or GED.

• You—or your child—has experienced abuse or neglect.

Please complete the following steps to request a Young Fives/Kindergarten Roundup appointment for your child and begin the registration process. After submitting the online forms, a member of the DES staff will reach out to you to set up an appointment for Roundup.

Please bring your child's original birth certificate, your child's immunization records, and proof of residency (such as a mortgage/lease statement, voter registration document, or a utility bill) to your Roundup appointment.

The Monroe County Health Department is scheduled to be at Roundup to screen each child's hearing, vision, and dental health. If this has recently been done by your child's physician/dentist, please bring a copy of those test results with you.

• For 2026-27 Kindergarten Roundup registration, complete the Pre-registration Google Form to schedule a registration appointment

• Complete the Online Kindergarten Roundup registration form in Infinite Campus

• Complete the DES Enrollment Questionnaire

• Remember to bring your child’s original birth certificate. We will make a copy for our records. 

• Provide a copy of your child's immunization records.

• Submit one proof of residency (such as a utility bill, driver’s license, etc.)

2. Complete the DES Enrollment Questionnaire

3. Complete the Transportation/Bus Form

4. Complete the DES Records Request Form

5. Provide your child’s original birth certificate. We will make a copy for our records. 

6. Provide a copy of your child's immunization records.

7. Submit one proof of residency (such as a utility bill, driver’s license, etc.)
